A RETROSPETIVE CLINICAL STUDY OF BASAL CELL CARCINOMA

Abstract
During a period of ten years from September 1977 thru August 1987, 102 patients with basal cell carcinoma were diagnosed at Yonsei University Medical Center. Twenty cases of recurrence occured after initial treatment. The authors reviewed the cases of recurrence with emphasis on clinical and histological findings and report the results obtained.

1. Of the total 102 patients, forth-eitht were male and fifty-four were female (M:F=1:1.13). The age distribution were as follows; thirty-two patients were in their fifties (31%), thirty patients were in their sixties (29%) and the majority of patients (89%) were over forty years of age.

2. The 102 patients had a total of 109 lesions, and the face was the most frequent site with 95 lesions (87%). The nose and eyelid were the most frequent sites of occurence with 20 lesions (27%) and 24 lesions (22%) respectively.

3. The morbidity period was more then three years in sixty-one (60%) patient average being 6.7 years.

4. The method of treatment consisted of excision and repair, excision and skin grafting, and excision and coverage with local flap in 102 lesions (95%). Electrocauterization, 5-Fluorouracil, and curettage were used in the treatment of 7 lesions (5%).

5. When treated with excision and repair in forty-two cases, its showed the highest rate of recurrence with ten cases (24%). A single case (3%) of recurrence occured out of a total of 37 lesions treated with skin grafts, and of the total of twenty-cases treated with local flaps, recurrence occured in two cases (10%).

6. among the recurrence cases, nose was the most frequent recurrence site (35%) and scalp and frontotemporal (24%) eyelid (17%), trunk (17%) and lower face (9%) were in order of frequency.

7. Of the twenty recurred cases that were treted, a second recurrence occured (25%), and among them three received more than three operations.

8. Pathologically incomplete excisions were found in four of the twenty recurred cases, and were observed conservatively without surgery, with recurrence later occuring in all. The second recurrence occured after six months in two cases, and after three and seven years in the remaining two respectively.
